Sepsis Readmission Prevention for Hospitals

Overview
This program supports patients discharged from the hospital after sepsis or infection as they transition safely back home. Through simple, real-life examples and everyday decision moments, patients learn how to recognize early warning signs of infection or sepsis and understand when to seek help. The focus is on empowering patients to act early, prevent complications, and reduce the risk of returning to the hospital during the critical first 30 days after discharge.
- Know the warning signs: Learn symptoms such as fever, chills, fast breathing, confusion, extreme weakness, or worsening pain that may signal infection or sepsis
- Daily recovery awareness: Understand the importance of medications, hydration, wound or infection care, and follow-up appointments
- When to get help: Practice knowing when and how to contact the care team or seek urgent care if symptoms worsen
