Knowing that diabetes education does not stop once the patient is discharged, Rose identified points in the continuum of care to reinforce diabetes education. By collaborating with a post acute rehab center and outpatient transplant clinics, patients and families continued to receive diabetes education along with endocrinology follow-up. These interventions prevented patients from being readmitted for hyperglycemia. One of the reasons Rose pursued the role of the CNS is her passion for education and the ethical responsibility a CNS has to transfer knowledge to improve lives. She knows knowledge is a tool that can empower patients and families and lead to improved quality of life. In addition to her work in patient education, Rose is a much sought after preceptor, mentor, presenter and lecturer for numerous disciplines. Teaching in venues as diverse as staff meetings, schools of nursing and medicine along with national and international organizations. Rose’s contributions as a Diabetes CNS earned her the 2022 Daisy Lifetime Achievement Award.
